The notification includes the product category and name, the name and address of the Responsible Person where the Product Information File is available, the country of origin for imported products, the member state where it is placed on the market, a contact person, nanomaterials, CMR substances of category 1A or 1B, and the frame formulation or exact formulation.
The original label and, where reasonably legible, a photograph of the packaging are also uploaded.
